When is a Scleral-Fixated IOL Needed?
Ideally, cataract surgery should place an intraocular lens (IOL) within the capsular bag. However, a pre-existing zonular deficit or an intraoperative posterior capsular tear can prevent this, resulting in aphakia — which requires an alternate means of IOL implantation.
Recent studies report no significant difference in visual recovery or complication rates between the three options. Each should be analysed by the surgeon's experience and the patient's age and comorbidities.

A Favoured Approach
A scleral-fixated IOL (SFIOL) involves sutured or sutureless fixation of the IOL to the sclera. Its position closely approximates the normal anatomic position of a within-the-bag IOL, and it does not hinder pupillary dilation, unlike an iris-claw IOL.
These factors make SFIOL a favoured IOL over the other two — although it is surgically more challenging and requires a certain amount of dexterity and skill.
